neutral un-rotated shoulder is present when the forearm is parallel to the centerline of the body

The shoulder internally rotates when the distal portion of the arm (the wrist) moves toward the centerline of the body

The shoulder externally rotates when the distal portion of the arm (the wrist) moves away from the centerline of the body
